Women whose breasts are as big as Kate's but you can tell at first glance that they have something inside, the surgery was done either under the breast or at the edge of the aeolas. Then you can see scars there, but Kate doesn't have those. The implants sit above the pectoral muscle then and even a blind man can recognize that something is wrong. However, if the implants are placed through the armpit, the pads sit under the pectoral muscle and the breast still looks natural. If you look closely at Kate, you can see scars in her armpits and sometimes when she lifts up her melons you can see fine indentations in the breast and gradations underneath. This would not be the case if she didn't have anything inside.
